Refer a patient - Pulmonary Rehab

St Vincent's Hospital provides a comprehensive pulmonary rehabilitation program to support people with chronic lung disease or experience shortness of breath. Pulmonary rehabilitation helps people with chronic lung disease return to active and productive lives.

Referrals can be made to the St Vincent's Pulmonary Rehabilitation program by a:

• GP
• Pulmonary Rehabilitation Service
• Respiratory Physician
• Thoracic surgeon.

How to make a referral

Please complete the St Vincent's Pulmonary Rehabilitation Referral Form and email it to SVHS.PulmRehab@svha.org.au

Following receipt of the completed form, a staff member will contact the participant to confirm an appointment.
